A physician directory (Polk/Dallas/Warren county) is available from the Polk County Medical Society. The directory lists alphabetically the doctor’s address, specialty, phone and fax numbers. The directory also lists frequently used phone numbers of hospitals, pharmacies, ambulance, and nursing homes.
